• The occupational safety card sample is specified in detail in form 06 appendix II of decree 44/2016/ND-CP
• Workers in group 3 will be issued with an occupational safety card (commonly known as occupational safety certificate for group 3).
• Accordingly, the card has a size of 60 mm x 90 mm, with the front and back contents shown as in the picture below.

The employer can issue group 3 safety cards to the trainees after they have passed the required assessment and there is a labor contract between the two parties.

• The training attendance logbook sample for groups 1, 2, 5, 6 is specified in detail in form 09 appendix II of decree 44/2016/ND-CP.
• The training attendance logbook sample for group 3 is specified in detail in form 10 appendix II of decree 44/2016/ND-CP
• The training attendance logbook sample for group 4 is specified in detail in form 11 appendix II of decree 44/2016/ND-CP
• For group 4, only the logbook is issued without a group 4 safety certificate.
