Occupational safety and hygiene training certificate

• The certificate template follows Form 08, Appendix II of Decree 44/2016/ND-CP and is issued to Groups 1, 2, 5, and 6.
• The certificate size is 13 cm x 19 cm with a sky blue cover.

Occupational Safety Card

• The occupational safety card template follows Form 06, Appendix II of Decree 44/2016/ND-CP
• Group 3 workers will be issued an Occupational Safety Card (commonly called Occupational Safety Certificate for Group 3).
• The card size is 60 mm x 90 mm, with front and back content shown below.

The employer may issue a Group 3 safety card to the trained worker after passing the assessment and having a labor contract between both parties.

Training Record Book

• Training record book for Groups 1, 2, 5, and 6 follows Form 09, Appendix II of Decree 44/2016/ND-CP.
• Training record book for Group 3 follows Form 10, Appendix II of Decree 44/2016/ND-CP.
• Training record book for Group 4 follows Form 11, Appendix II of Decree 44/2016/ND-CP.
• For Group 4, only a record book is issued without a Group 4 safety certificate.
