The employer can issue a Group 3 safety card to the trained employee after a successful evaluation and if a labor contract exists between both parties.

• The training records book format for groups 1, 2, 5, 6 follows Template 09, Appendix II, Decree 44/2016/ND-CP.
• The training records book for group 3 follows Template 10, Appendix II, Decree 44/2016/ND-CP.
• The training records book for group 4 follows Template 11, Appendix II, Decree 44/2016/ND-CP.
