• The certificate template follows Form 08, Appendix II of Decree 44/2016/ND-CP, and is issued to Groups 1,2,5,6.

Occupational safety card

• The card template follows Form 06, Appendix II of Decree 44/2016/ND-CP
• Group 3 workers are issued an occupational safety card (commonly called occupational safety certificate Group 3).

The employer may issue Group 3 safety cards to trained workers after passing the assessment and having a labor contract between the two parties.

• Group 1, 2, 5, 6 logbooks follow Form 09, Appendix II of Decree 44/2016/ND-CP
• Group 3 logbooks follow Form 10, Appendix II of Decree 44/2016/ND-CP
• Group 4 logbooks follow Form 11, Appendix II of Decree 44/2016/ND-CP
• Group 4 only receives a logbook without Group 4 safety certificate.
