Occupational safety and health training certificate

• The template for the occupational safety and health training certificate is specified in form 08, appendix II of decree 44/2016/ND-CP and they are issued for groups 1, 2, 5, 6.
• Accordingly, the certificate has a size of 13 cm x 19 cm, its outer side is sky blue.

Occupational safety card

• The template for the occupational safety card is specified in detail in form 06, appendix II of decree 44/2016/ND-CP
• Group 3 employees will be issued an occupational safety card (in practice, also called the occupational safety certificate for group 3).
• Accordingly, the card has a size of 60 mm x 90 mm, the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the trained person after they pass the exam and there is an employment contract between the two parties.

Logbook of trained persons

• The template for the logbook of trained persons for groups 1, 2, 5, 6 is specified in detail in form 09, appendix II of decree 44/2016/ND-CP.
• The template for the logbook of trained persons for group 3 is specified in detail in form 10, appendix II of decree 44/2016/ND-CP.
• The template for the logbook of trained persons for group 4 is specified in detail in form 11, appendix II of decree 44/2016/ND-CP.
• Group 4 is only issued a logbook and does not have a group 4 safety certificate.
