Certificate of occupational safety and hygiene training

• The template for the certificate of occupational safety and hygiene training is described in detail at template 08, appendix II of Decree 44/2016/ND-CP and they are issued for groups 1, 2, 5, and 6.
• Accordingly, the certificate template has a size of 13 cm x 19 cm, and its outer surface is sky blue.

Occupational safety card

• The template for the occupational safety card is described in detail at template 06, appendix II of Decree 44/2016/ND-CP
• Group 3 workers will be issued an occupational safety card (in practice, often called a group 3 occupational safety certificate).
• Accordingly, the card template has a size of 60 mm x 90 mm, and the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the trained employee after they pass the required test and there is an **employment contract** between the two parties.

Training logbook for trained individuals

• The template for the training logbook for trained individuals in groups 1, 2, 5, and 6 is described in detail at template 09, appendix II of Decree 44/2016/ND-CP.
• The template for the training logbook for trained individuals in group 3 is described in detail at template 10, appendix II of Decree 44/2016/ND-CP

• Specifically for group 4, only a logbook is issued, not a **group 4 safety certificate**.
