Occupational Safety Card

• The safety card template is described in detail in Form 06, Appendix II of Decree 44/2016/ND-CP
• Workers in Group 3 will be issued a safety card (commonly referred to as Group 3 safety certificate).
• The card has a size of 60 mm x 90 mm, with front and back content shown below.

Employers may issue a Group 3 safety card to a trainee after passing the safety training and having a labor contract between the two parties.

• The monitoring book template for Groups 1, 2, 5, 6 is described in Form 09, Appendix II of Decree 44/2016/ND-CP.
