• The sample occupational safety and hygiene training certificate is specified in Form 08, Appendix II of Decree 44/2016/ND-CP and is issued to groups 1, 2, 5, 6.
• Accordingly, the sample certificate has a size of 13 centimeters x 19 centimeters, with a sky blue cover.

Occupational safety card

• The occupational safety card sample is specified in detail in Form 06, Appendix II of Decree 44/2016/ND-CP.
• Group 3 workers will be issued an occupational safety card (commonly referred to as occupational safety certificate group 3).
• Accordingly, the card sample has a size of 60 millimeters x 90 millimeters, and the contents on the front and back sides are shown as below.

The employer may issue a group 3 safety card to the person trained in occupational safety after passing the required test and having a labor contract between the two parties.

Training monitoring book

• The training monitoring book for groups 1, 2, 5, 6 is specified in detail in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The training monitoring book for group 3 is specified in detail in Form 10, Appendix II of Decree 44/2016/ND-CP.

• Specifically for group 4, only the monitoring book is issued, without a group 4 safety certificate.
