• The card size is 60 mm x 90 mm, with front and back contents as shown below.

The employer may issue a group 3 safety card to the trained employee after verifying requirements are met and there is a labor contract between both parties.

• The logbook template for groups 1, 2, 5, 6 is described in Form 09, Appendix II of Decree 44/2016/ND-CP.
