Labor safety training certificate

• The sample labor safety training certificate is specified in Form 08, Appendix II of Decree 44/2016/ND-CP and issued to groups 1, 2, 5, and 6.
• Accordingly, the certificate measures 13 cm x 19 cm, with a sky-blue cover.

Labor safety card

• The labor safety card is specified in detail in Form 06, Appendix II of Decree 44/2016/ND-CP
• Workers in Group 3 will be granted a labor safety card (commonly referred to as a labor safety certificate for Group 3).
• The card measures 60 mm x 90 mm, with content displayed on both sides as shown below.

The employer may issue a Group 3 labor safety card to trained workers after they pass the assessment and have a labor contract with the employer.

Training record book

• The training record book for Groups 1, 2, 5, and 6 is specified in detail in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The record book for Group 3 is specified in Form 10, Appendix II of Decree 44/2016/ND-CP
• The record book for Group 4 is specified in Form 11, Appendix II of Decree 44/2016/ND-CP
• For Group 4, only a record book is issued, without a Group 4 safety certificate.
