The employer can issue a group 3 occupational safety card to the trained employee after passing the requirements and having a labor contract between the two parties.

• The training record book for groups 1, 2, 5, and 6 is described in detail in Form 09, Appendix II of Decree 44/2016/ND-CP.

• For group 4 only the record book is issued without an occupational safety certificate for group 4.
