• Group 5: 16 hours

d. Occupational Safety Training Certificate

Occupational Safety and Hygiene Training Certificate

• The certificate sample is specified in Form 08, Appendix II of Decree 44/2016/ND-CP, issued to Groups 1, 2, 5, 6.
• The certificate size is 13 cm x 19 cm with a light blue front cover.

Occupational Safety Card

• The safety card sample is specified in Form 06, Appendix II of Decree 44/2016/ND-CP.
• Workers in Group 3 are issued Occupational Safety Card (commonly known as Group 3 Safety Certificate).
• The card size is 60 mm x 90 mm with front and back content as shown below.

Employers can issue Group 3 safety cards to trained workers after passing assessment and having an employment contract between both parties.

Training Record Book

• Form 09, Appendix II specifies record books for Groups 1, 2, 5, 6.
• Form 10 specifies record books for Group 3.
• Form 11 specifies record books for Group 4.
• Group 4 only receives record books without Group 4 Safety Certificate.
