• The certificate is 13 cm x 19 cm in size, with a sky-blue cover.

Occupational safety card

• The occupational safety card template follows the specifications in Form 06, Appendix II of Decree 44/2016/ND-CP
• Group 3 workers will be issued an occupational safety card (commonly referred to as Group 3 occupational safety certificate).
• The card size is 60 mm x 90 mm, with front and back content as shown below.

The employer may issue a Group 3 safety card to the trainee after verification and upon having a labor contract between both parties.

Trainee tracking book

• The trainee tracking book template for groups 1, 2, 5, 6 follows Form 09, Appendix II of Decree 44/2016/ND-CP.
• The trainee tracking book template for group 3 follows Form 10, Appendix II of Decree 44/2016/ND-CP.
• The trainee tracking book template for group 4 follows Form 11, Appendix II of Decree 44/2016/ND-CP.
• For group 4, only a tracking book is issued, without a Group 4 safety certificate.
