Occupational safety card

• The sample occupational safety card is specified in detail in Form 06 of Appendix II of Decree 44/2016/ND-CP
• Employees in group 3 will be issued an occupational safety card (commonly called occupational safety certificate for group 3).
• Accordingly, the card sample has a size of 60 mm x 90 mm, with the front and back content shown as in the image below.

The employer may issue a group 3 safety card to the employee who has completed the occupational safety training after passing the assessment and having a labor contract between the two parties.

Training monitoring book

• The sample monitoring book for groups 1, 2, 5, 6 is specified in detail in Form 09 of Appendix II of Decree 44/2016/ND-CP.
• The sample monitoring book for group 3 is specified in detail in Form 10 of Appendix II of Decree 44/2016/ND-CP
• The sample monitoring book for group 4 is specified in detail in Form 11 of Appendix II of Decree 44/2016/ND-CP
• For group 4, only the monitoring book is issued without a group 4 safety certificate.
