- Group 3 employees are issued a safety card (commonly known as occupational safety certificate Group 3).
- The card measures 60 mm x 90 mm, with the front and back content as shown below.

The employer may issue a Group 3 safety card to the trained employee after verifying satisfactory performance and having a labor contract between both parties.
