d. Occupational safety training certificate

Occupational safety and hygiene training certificate

• The occupational safety and hygiene training certificate template is specified in template 08 appendix II Decree 44/2016/ND-CP and is issued for groups 1, 2, 5, 6.
• Accordingly, the certificate has a size of 13 cm x 19 cm, and its cover is sky blue.

Occupational safety card

• The occupational safety card template is specified in detail in template 06 appendix II Decree 44/2016/ND-CP
• Employees in group 3 will be issued an occupational safety card (commonly referred to as occupational safety certificate group 3).
• Accordingly, the card has a size of 60 mm x 90 mm, and the content on the front and back sides is shown as below.

The employer may issue group 3 occupational safety cards to trained employees after passing the required test and having a labor contract between the two parties.

Training monitoring book

• The training monitoring book for groups 1, 2, 5, 6 is specified in detail in template 09 appendix II Decree 44/2016/ND-CP.
• The training monitoring book for group 3 is specified in detail in template 10 appendix II Decree 44/2016/ND-CP
• The training monitoring book for group 4 is specified in detail in template 11 appendix II Decree 44/2016/ND-CP
• For group 4 only the training monitoring book is issued, without an occupational safety certificate group 4.
