Employers may issue Group 3 Occupational Safety Cards to trained employees after verification and having a labor contract.

• Form 09, Appendix II, Decree 44/2016/ND-CP applies to Groups 1, 2, 5, 6.
• Form 10 applies to Group 3, Form 11 to Group 4.
