Employers can issue Group 3 safety cards after verifying that the trainee meets requirements and has a labor contract in place.

Training Tracking Log

• Forms for tracking Groups 1, 2, 5, 6 follow Form 09; Group 3 follows Form 10; Group 4 follows Form 11 of Appendix II, Decree 44/2016/ND-CP.
• Group 4 receives only a tracking log without a certificate.
