d. Labor safety training certificate

Labor safety and hygiene training certificate

• The template for the labor safety and hygiene training certificate is detailed in form 08, appendix II of decree 44/2016/ND-CP and they are issued to groups 1,2,5,6.
• Accordingly, the certificate template has a size of 13 cm x 19 cm, its outer side has a sky blue color.

Labor safety card

• The template for the labor safety card is detailed in form 06, appendix II of decree 44/2016/ND-CP
• Group 3 workers will be issued a labor safety card (in practice, it is often called a Group 3 labor safety certificate).
• Accordingly, the card template has a size of 60 mm x 90 mm, the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the trained person after they pass the test and have a labor contract between the two sides.

Trainee tracking book

• The template for the trainee tracking book for groups 1, 2, 5, 6 is detailed in form 09, appendix II of decree 44/2016/ND-CP.
• The template for the trainee tracking book for group 3 is detailed in form 10, appendix II of decree 44/2016/ND-CP
• The template for the trainee tracking book for group 4 is detailed in form 11, appendix II of decree 44/2016/ND-CP
• Group 4 is only issued a tracking book without a group 4 safety certificate.
• The content in the group 4 tracking book is shown as in the image below.
