• Group 2: 48 hours

• Group 4: 16 hours
• Group 5: 16 hours

d. Occupational safety training certificate

Certificate of occupational safety and hygiene training

• The sample certificate of occupational safety and hygiene training is described in detail in form 08 appendix II of decree 44/2016/ND-CP and they are issued for groups 1, 2, 5, 6.
• Accordingly, the certificate has a size of 13 cm x 19 cm, and its outer surface is sky blue.

Occupational safety card

• The sample occupational safety card is described in detail in form 06 appendix II of decree 44/2016/ND-CP
• Group 3 employees will be issued an occupational safety card (in reality often called a group 3 occupational safety certificate).
• Accordingly, the card has a size of 60 mm x 90 mm, and the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the trained person after they pass the test and if there is a labor contract between the two parties.

Record book of trained persons

• The sample record book of trained persons for groups 1, 2, 5, 6 is described in detail in form 09 appendix II of decree 44/2016/ND-CP.
• The sample record book of trained persons for group 3 is described in detail in form 10 appendix II of decree 44/2016/ND-CP
• The sample record book of trained persons for group 4 is described in detail in form 11 appendix II of decree 44/2016/ND-CP
• Specifically for group 4, only a record book is issued, and not an occupational safety certificate for group 4.

3. Employment issues in Cao Bang
