• The labor safety card follows Form 06, Appendix II, Decree 44/2016/ND-CP.

Employers can issue a Group 3 labor safety card to trained employees after they meet requirements and have an employment contract between both parties.

Training record book

• The training record book for groups 1, 2, 5, and 6 follows Form 09, Appendix II, Decree 44/2016/ND-CP.
• The training record book for group 3 follows Form 10, Appendix II, Decree 44/2016/ND-CP.
• The training record book for group 4 follows Form 11, Appendix II, Decree 44/2016/ND-CP.
• For group 4, only a record book is issued, not a Group 4 safety certificate.
