Occupational safety training certificate

• Accordingly, the certificate template has a size of 13 cm x 19 cm, and its outer surface is sky blue.

Occupational safety card

• The occupational safety card template is described in detail in template 06, appendix II of decree 44/2016/ND-CP
• Group 3 employees will be issued an occupational safety card (commonly referred to as a Group 3 occupational safety certificate ).
• Accordingly, the card template has a size of 60 mm x 90 mm, and the content on the front and back is shown as in the image below.

The employer can issue a Group 3 safety card to the occupational safety trained person after they pass the exam and there is a labor contract between the two parties.

Trainee logbook

• The template for the trainee logbook for groups 1, 2, 5, and 6 is described in detail in template 09, appendix II of decree 44/2016/ND-CP.

• Only group 4 is issued a logbook and not a Group 4 safety certificate.
