d. Occupational safety training certificate

Occupational safety and health training certificate

• The form of the occupational safety and health training certificate is described in detail in form 08, appendix II of decree 44/2016/ND-CP and they are issued to groups 1,2,5,6.
• Accordingly, the certificate form has a size of 13 cm x 19 cm, its outer surface is sky blue.

Occupational safety card

• The form of the occupational safety card is described in detail in form 06, appendix II of decree 44/2016/ND-CP
• Group 3 employees will be issued an occupational safety card (in reality often called a group 3 safety certificate).
• Accordingly, the card form has a size of 60 mm x 90 mm, the content on the front and back is shown in the image below.

The employer can issue a group 3 safety card to the trained person after a successful examination and a labor contract between the two parties.

Trained person’s monitoring book

• The form of the trained person’s monitoring book for groups 1, 2, 5, 6 is described in detail in form 09, appendix II of decree 44/2016/ND-CP.
• The form of the trained person’s monitoring book for group 3 is described in detail in form 10, appendix II of decree 44/2016/ND-CP
• The form of the trained person’s monitoring book for group 4 is described in detail in form 11, appendix II of decree 44/2016/ND-CP
• Especially for group 4, only a monitoring book is issued, not a group 4 safety certificate.
