Occupational Safety and Hygiene Training Certificate

• The certificate measures 13 cm x 19 cm, with a sky blue cover.

Occupational Safety Card

• The card template is described in detail in Form 06, Appendix II of Decree 44/2016/ND-CP.
• Group 3 workers are issued an Occupational Safety Card (commonly referred to as Group 3 Safety Certificate).
• The card measures 60 mm x 90 mm, with front and back content shown as below.

The employer may issue a Group 3 Safety Card to trainees after verifying that they meet the requirements and have a labor contract between both parties.

Trainee Monitoring Book

• Monitoring book templates for groups 1, 2, 5, and 6 are detailed in Form 09, Appendix II of Decree 44/2016/ND-CP.
• Group 3 monitoring book template is detailed in Form 10, Appendix II.
• Group 4 monitoring book template is detailed in Form 11, Appendix II.
• Group 4 only receives a monitoring book without a Group 4 Safety Certificate.

3. Employment situation in Hai Duong
