• The sample of the occupational safety and hygiene training certificate is specified in Form 08, Appendix II, Decree 44/2016/ND-CP, and is issued to groups 1, 2, 5, 6.
• Accordingly, the certificate has dimensions of 13 cm x 19 cm, with a sky-blue cover.

Occupational safety card

• The occupational safety card sample is detailed in Form 06, Appendix II, Decree 44/2016/ND-CP.
• Group 3 workers will be issued an occupational safety card (commonly referred to as occupational safety certificate for group 3).
• Accordingly, the card has dimensions of 60 mm x 90 mm, with the front and back content shown below.

The employer may issue a group 3 safety card to employees after they pass the required safety training assessment and have a labor contract between both parties.

Training monitoring book

• The training monitoring book for groups 1, 2, 5, 6 is detailed in Form 09, Appendix II, Decree 44/2016/ND-CP.
• The training monitoring book for group 3 is detailed in Form 10, Appendix II, Decree 44/2016/ND-CP.
• The training monitoring book for group 4 is detailed in Form 11, Appendix II, Decree 44/2016/ND-CP.
• Specifically, group 4 is only issued a monitoring book and not a group 4 safety certificate.
