Occupational safety and hygiene training certificate

• The sample occupational safety and hygiene training certificate is described in detail in sample 08 Appendix II Decree 44/2016/NĐ-CP and issued to groups 1, 2, 5, 6.
• Accordingly, the certificate sample has a size of 13 cm x 19 cm, with the outside cover in sky blue color.

Occupational safety card

• The occupational safety card sample is described in detail in sample 06 Appendix II Decree 44/2016/NĐ-CP
• Group 3 workers will be issued an occupational safety card (commonly referred to as occupational safety certificate for group 3).
• Accordingly, the card sample has a size of 60 mm x 90 mm, with the front and back content shown as in the image below.

The employer may issue group 3 safety cards to trained workers after passing the required test and having a labor contract between both parties.

Training record book

• The training record book for groups 1, 2, 5, 6 is described in detail in sample 09 Appendix II Decree 44/2016/NĐ-CP.

• Specifically for group 4, only a training record book is issued without a group 4 safety certificate.
