Occupational safety card

• The occupational safety card format is detailed in Form 06, Appendix II, Decree 44/2016/ND-CP.
• Group 3 employees are issued the occupational safety card (commonly referred to as Group 3 occupational safety certificate).
• The card size is 60 mm x 90 mm, with front and back displayed as shown below.

The employer may issue a Group 3 occupational safety card to the trained employee after verifying compliance and having a labor contract between the parties.

• The training attendance book for Groups 1, 2, 5, 6 is detailed in Form 09, Appendix II, Decree 44/2016/ND-CP.
