Occupational Safety Card

• The occupational safety card template follows Form 06, Appendix II of Decree 44/2016/ND-CP.
• Employees in group 3 will be issued an occupational safety card (commonly called group 3 safety certificate).
• The card measures 60 mm x 90 mm, with front and back content as shown in the image below.

The employer may issue a group 3 occupational safety card to the trained employee after passing the assessment and having a labor contract between the two parties.

Training Record Book

• The training record book for groups 1, 2, 5, and 6 follows Form 09, Appendix II of Decree 44/2016/ND-CP.
• The training record book for group 3 follows Form 10, Appendix II of Decree 44/2016/ND-CP.
• The training record book for group 4 follows Form 11, Appendix II of Decree 44/2016/ND-CP.
• Group 4 employees are only issued a record book, not a group 4 safety certificate.
