Occupational safety and hygiene training certificate

• The sample occupational safety and hygiene training certificate is specified in Form 08 Appendix II of Decree 44/2016/ND-CP and is issued to groups 1, 2, 5, 6.
• Accordingly, the sample certificate has a size of 13 cm x 19 cm, and its outer side is sky blue.

Occupational safety card

• The sample occupational safety card is specified in detail in Form 06 Appendix II of Decree 44/2016/ND-CP
• Group 3 workers will be issued an occupational safety card (commonly referred to as occupational safety certificate for group 3).
• Accordingly, the sample card has a size of 60 mm x 90 mm, and the contents of the front and back sides are shown as below.

The employer may issue a group 3 safety card to the trained employee after passing the required test and having a labor contract between the two parties.

Training monitoring book

• The training monitoring book for groups 1, 2, 5, 6 is specified in detail in Form 09 Appendix II of Decree 44/2016/ND-CP.
• The training monitoring book for group 3 is specified in detail in Form 10 Appendix II of Decree 44/2016/ND-CP
• The training monitoring book for group 4 is specified in detail in Form 11 Appendix II of Decree 44/2016/ND-CP
• For group 4, only the monitoring book is issued and there is no group 4 safety certificate.
