Occupational safety and hygiene training certificate

• The template for the occupational safety and hygiene training certificate is described in detail in template 08, appendix II of decree 44/2016/ND-CP, and they are issued to groups 1, 2, 5, 6.
• Accordingly, the certificate has a size of 13 cm x 19 cm, and its outer surface is sky blue.

Occupational safety card

• The template for the occupational safety card is described in detail in template 06, appendix II of decree 44/2016/ND-CP.
• Group 3 employees will be issued an occupational safety card (in practice, also called the group 3 occupational safety certificate).
• Accordingly, the card has a size of 60 mm x 90 mm, and the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the person who has been trained in occupational safety after they pass the test and there is a labor contract between the two parties.

Logbook of trained persons

• The template for the logbook of trained persons for groups 1, 2, 5, 6 is described in detail in template 09, appendix II of decree 44/2016/ND-CP.
• The template for the logbook of trained persons for group 3 is described in detail in template 10, appendix II of decree 44/2016/ND-CP.
• The template for the logbook of trained persons for group 4 is described in detail in template 11, appendix II of decree 44/2016/ND-CP.
• Specifically for group 4, they are only issued a logbook and not a group 4 safety certificate.
