Occupational safety and hygiene training certificate

• The occupational safety and hygiene training certificate template is specified in template 08, appendix II of decree 44/2016/ND-CP and is issued to groups 1, 2, 5, 6.
• Accordingly, the certificate template has a size of 13 cm x 19 cm, and its outer surface is sky blue.

Occupational safety card

• The occupational safety card template is described in detail in template 06, appendix II of decree 44/2016/ND-CP
• Employees in group 3 will be issued an occupational safety card (in practice, it is often called the occupational safety certificate for group 3).
• Accordingly, the card template has a size of 60 mm x 90 mm, the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the trained employee after the employee passes the test and there is a labor contract between the two parties.

Logbook of trained people

• Specifically for group 4, only a logbook is issued, not an occupational safety certificate for group 4.
