An Phat Industrial Park is an industrial park located in Tu Minh Commune, Hai Duong City, Hai Duong, Vietnam. This industrial park was established in 2007 by An Phat High-Tech Complex Company Limited (An Phat Complex).

Before establishing An Phat Industrial Park, An Phat Group had invested in several plastic production projects in Vietnam. Around 2005, An Phat Group decided to expand its production operations by investing in a new industrial park in Hai Duong.

After extensive negotiations and legal procedures, An Phat Group was officially granted an investment license for An Phat Industrial Park in August 2007. The industrial park has a total area of 46 hectares, of which 30 hectares are allocated for plastic production factories, and 16 hectares are for infrastructure and support services.

Since its establishment, An Phat Industrial Park has attracted many domestic and foreign investors. Occupational safety and hygiene training certificate

• The template for the occupational safety and hygiene training certificate is described in Form 08, Appendix II of Decree 44/2016/ND-CP, and is issued for groups 1, 2, 5, and 6.
• The certificate size is 13 cm x 19 cm, with a sky-blue front cover.

Occupational safety card

• The occupational safety card template is described in detail in Form 06, Appendix II of Decree 44/2016/ND-CP
• Employees in Group 3 are issued an occupational safety card (commonly referred to as Group 3 safety certificate).
• The card measures 60 mm x 90 mm, with front and back content displayed as below.

The employer may issue a Group 3 safety card to the trained employee after verification and with a labor contract between the two parties.

Trainee monitoring log

• The monitoring log template for trainees in groups 1, 2, 5, 6 is detailed in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The monitoring log for Group 3 trainees is detailed in Form 10, Appendix II of Decree 44/2016/ND-CP.
• The monitoring log for Group 4 trainees is detailed in Form 11, Appendix II of Decree 44/2016/ND-CP.
• For Group 4, only a monitoring log is issued, without a Group 4 safety certificate.
