• Group 3: 24 hours

• Group 5: 16 hours

d. Occupational safety training certificate

• The certificate sample is described in Form 08, Appendix II of Decree 44/2016/ND-CP and is issued for groups 1, 2, 5, 6.
• The certificate size is 13 cm x 19 cm, with a light blue outer cover.

Occupational Safety Card

• The card design is detailed in Form 06, Appendix II of Decree 44/2016/ND-CP.
• Workers in Group 3 will be issued the Occupational Safety Card (commonly called Group 3 Safety Certificate).
• The card size is 60 mm x 90 mm, with front and back details displayed as shown below.

The employer may issue a Group 3 safety card to the trained employee after passing the assessment and signing a labor contract between both parties.

Training Record Book

• The training record book for groups 1, 2, 5, 6 is described in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The training record book for Group 3 is described in Form 10, Appendix II of Decree 44/2016/ND-CP.
• The training record book for Group 4 is described in Form 11, Appendix II of Decree 44/2016/ND-CP.
• Group 4 is only issued a record book and not a Group 4 safety certificate.
