Occupational safety card

Employers can issue a group 3 safety card to the trained employee after passing assessment and having a labor contract between the parties.

• The training record book template for groups 1, 2, 5, 6 is described in form 09, Appendix II, Decree 44/2016/ND-CP.
• For group 3, form 10 is used.
• For group 4, form 11 is used.
• Group 4 receives only the record book without a group 4 safety certificate.
