Occupational Safety Card

• The occupational safety card template is detailed in Form 06, Appendix II of Decree 44/2016/ND-CP
• Group 3 workers are issued occupational safety cards (commonly called Group 3 safety certificates).
• The card measures 60 mm x 90 mm, with front and back content displayed as below.

The employer may issue a Group 3 occupational safety card to the trained individual after passing the assessment and having a labor contract between both parties.

Trainee tracking book

• The tracking book for trainees in groups 1, 2, 5, 6 is detailed in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The tracking book for group 3 trainees is detailed in Form 10, Appendix II of Decree 44/2016/ND-CP.
• The tracking book for group 4 trainees is detailed in Form 11, Appendix II of Decree 44/2016/ND-CP.
• For group 4, only the tracking book is issued, no Group 4 safety certificate.
