Occupational Safety Card

Employers can issue Group 3 safety cards to workers after they successfully complete training and have a labor contract.

• Logbooks for Groups 1, 2, 5, 6 follow Form 09, Appendix II, Decree 44/2016/ND-CP.
• Logbook for Group 3 follows Form 10, Appendix II, Decree 44/2016/ND-CP.
• Logbook for Group 4 follows Form 11, Appendix II, Decree 44/2016/ND-CP.
