Labor safety and hygiene training certificate

• The mẫu of the labor safety and hygiene training certificate is described in detail in Form 08, Appendix II of Decree 44/2016/ND-CP and they are issued to groups 1, 2, 5, 6.
• Accordingly, the certificate has a size of 13 cm x 19 cm, and its outer surface is sky blue.

Labor safety card

• The mẫu of the labor safety card is described in detail in Form 06, Appendix II of Decree 44/2016/ND-CP.
• Group 3 workers will be issued a labor safety card (also commonly called a Group 3 labor safety certificate).
• Accordingly, the card has a size of 60 mm x 90 mm, and the content on the front and back is shown in the image below.

The employer can issue a Group 3 safety card to the trained person after the inspection meets the requirements and there is a labor contract between the two parties.

Tracking logbook for trained individuals

• The mẫu of the tracking logbook for trained individuals in groups 1, 2, 5, 6 is described in detail in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The mẫu of the tracking logbook for trained individuals in Group 3 is described in detail in Form 10, Appendix II of Decree 44/2016/ND-CP.
• The mẫu of the tracking logbook for trained individuals in Group 4 is described in detail in Form 11, Appendix II of Decree 44/2016/ND-CP.
• Specifically, Group 4 is only issued a tracking logbook and not a Group 4 safety certificate.

• An example of a labor accident: A male worker fell from the second floor of a house under construction and died in Binh Duong: According to local residents, in the morning of the same day, a group of construction workers were working at a residential construction site on Le Hong Phong street, Tan Dong Hiep ward, when they suddenly heard a loud noise coming from the second floor. At this time, the workers ran to check and found Mr. N.T.H. (58 years old, residing in Di An City) lying motionless on the ground and took the victim to the emergency room but he did not survive. The authorities were present to check legal documents and determined that the above project was a residential house under construction up to the 3rd floor when the incident occurred.
