Occupational safety card

• The occupational safety card template is detailed in Form 06 of Appendix II of Decree 44/2016/ND-CP.
• Group 3 workers will be issued an occupational safety card (commonly referred to as occupational safety certificate for group 3).
• Accordingly, the card has a size of 60 mm x 90 mm, and the content on the front and back sides is presented as shown below.

The employer can issue the group 3 safety card to the trained worker after passing the assessment and having a labor contract between the two parties.

Training monitoring book

• The training monitoring book for groups 1, 2, 5, and 6 is detailed in Form 09 of Appendix II of Decree 44/2016/ND-CP.
• The training monitoring book for group 3 is detailed in Form 10 of Appendix II of Decree 44/2016/ND-CP.
• The training monitoring book for group 4 is detailed in Form 11 of Appendix II of Decree 44/2016/ND-CP.
• For group 4, only the monitoring book is issued, without the group 4 safety certificate.
