Long Thanh industrial park is one of the major industrial parks in Dong Nai province, Vietnam. This industrial park was established in 2004 on an area of about 488 hectares, with the purpose of supporting the economic and industrial development of Dong Nai province.

Occupational safety and hygiene training certificate

• The sample occupational safety and hygiene training certificate is specified in form 08 of appendix II of decree 44/2016/ND-CP and is issued for groups 1, 2, 5, 6.
• Accordingly, the certificate has a size of 13 cm x 19 cm, and its cover is sky blue.

Occupational safety card

• The sample occupational safety card is specified in detail in form 06 of appendix II of decree 44/2016/ND-CP
• Group 3 employees will be issued an occupational safety card (commonly referred to as a group 3 occupational safety certificate).
• Accordingly, the card has a size of 60 mm x 90 mm, with content on both front and back as shown below.

The employer may issue a group 3 occupational safety card to the trainee after passing the required assessment and having a labor contract between both parties.

Training record book

• The training record book for groups 1, 2, 5, 6 is specified in detail in form 09 of appendix II of decree 44/2016/ND-CP.
• The training record book for group 3 is specified in detail in form 10 of appendix II of decree 44/2016/ND-CP
• The training record book for group 4 is specified in detail in form 11 of appendix II of decree 44/2016/ND-CP
• Specifically for group 4, only the training record book is issued without a group 4 safety certificate.
