Labor safety card

• The labor safety card template is described in detail in Form 06, Appendix II of Decree 44/2016/ND-CP.
• Employees in Group 3 will be issued a labor safety card (commonly referred to as a labor safety certificate for Group 3).
• The card measures 60 mm x 90 mm, with content shown on both front and back as below.

Employers may issue Group 3 labor safety cards to trainees after they pass the training assessment and sign a labor contract between the two parties.

Training monitoring logbook

• The logbook for Groups 1, 2, 5, 6 is described in detail in Form 09, Appendix II of Decree 44/2016/ND-CP.
• The logbook for Group 3 is described in detail in Form 10, Appendix II of Decree 44/2016/ND-CP.
• The logbook for Group 4 is described in detail in Form 11, Appendix II of Decree 44/2016/ND-CP.
• Specifically, Group 4 is only issued a monitoring logbook without a Group 4 safety certificate.
