Occupational safety and hygiene training certificate

• The certificate format is described in Form 08, Appendix II of Decree 44/2016/ND-CP and is issued to Groups 1, 2, 5, and 6.
• The certificate is 13 cm x 19 cm in size, with a sky blue cover.

Occupational safety card

• The card format is described in Form 06, Appendix II of Decree 44/2016/ND-CP
• Group 3 employees are issued a Group 3 occupational safety card (commonly called Group 3 safety certificate).
• The card is 60 mm x 90 mm, with front and back content displayed as shown below.

The employer may issue a Group 3 safety card to the trained employee after passing the test and having a labor contract between both parties.

Trainee monitoring book

• Form 09, Appendix II of Decree 44/2016/ND-CP describes the monitoring book for Groups 1, 2, 5, 6.
• Form 10 describes the monitoring book for Group 3.
• Form 11 describes the monitoring book for Group 4.
• Group 4 only receives a monitoring book and does not receive a Group 4 safety certificate.
