Occupational safety and hygiene training certificate

Occupational safety card

• The occupational safety card form is described in detailed specifications in form 06, appendix II, decree 44/2016/ND-CP
• Employees in group 3 will be issued an occupational safety card (in practice, it is often called a group 3 occupational safety certificate ).
• Accordingly, the card form has a size of 60 mm x 90 mm, the content on the front and back is shown as in the image below.

The employer can issue a group 3 safety card to the trained employee after the inspection meets the requirements and there is a labor contract between the two parties.

Tracking log for trained people

• The tracking log form for trained people in groups 1, 2, 5, 6 is described in detailed specifications in form 09, appendix II, decree 44/2016/ND-CP.
• The tracking log form for trained people in group 3 is described in detailed specifications in form 10, appendix II, decree 44/2016/ND-CP
• The tracking log form for trained people in group 4 is described in detailed specifications in form 11, appendix II, decree 44/2016/ND-CP
• Specifically for group 4, only a tracking log is issued, without a group 4 safety certificate.
