• The certificate format follows form 08, Appendix II, Decree 44/2016/ND-CP and is issued to Groups 1, 2, 5, 6.

Occupational safety card

• The safety card format is detailed in form 06, Appendix II, Decree 44/2016/ND-CP
• Group 3 employees are issued occupational safety cards (commonly called Group 3 safety certificates).

Employers may issue Group 3 safety cards to trained employees after passing the assessment and having a labor contract between the two parties.

• Record books for Groups 1, 2, 5, 6 follow form 09, Appendix II, Decree 44/2016/ND-CP.
